Please note that campers must be entering these grades in the fall of 2020 in order to qualify for attendance.
**In order to take advantage of the family rate, campers must be siblings in the same immediate family.

***A non-refundable registration fee of $100 is required at the time of application for each student.
This amount will be deducted from the full fee.

This year we are limited to the number of campers we are able to accept. We can accommodate 32 students. (16 girls/16 guys) on a first come first serve basis. We can not guarantee your spot until we have received your registration.

I understand that, should my child be involved in any unsupervised camp activities (i.e. late-night pranks, roughhousing) which cause damage to either the campground or the personal belongings of another camper or staff member, I will be responsible to make restitution for the full replacement or repair costs as soon as they are determined.

I have read this Informed Consent and Assumption of Risk Agreement, fully understand its terms and the risks I am assuming by signing it, and sign it freely and voluntarily.

I Understand *

I understand that boating, water sports, and other camp activities have inherent risks beyond the contemplation and control of camp staff. Precautions are taken for the health and safety of your child, but in the event of accidental death, injury or illness, True Life Church, its staff and volunteers are hereby released from any and all liability. I give the leadership of this camp the authority to make medical decisions in my absence on my child’s behalf. In the event that any camper requires special medication, x-rays or treatment, parents/guardians will be notified immediately. In case of a surgical emergency, I hereby give permission to the attending physician to hospitalize, secure proper treatment for, and to order anaesthesia or surgery for my child.
